When you join the scheme, your employer should provide you with a link to this page for the joiner pack. It contains 2 forms for you to complete:
Expression of wish for payment of death grant form
This form enables you to express who you wish to receive the death grant lump sum if you die in service. The death grant lump sum is 3 times your annual pensionable pay.

Personal details declaration form
This form enables you to:
- give us your personal details, and
- express an interest in a transfer of previous pension rights. A transfer can only be requested within 12 months of joining the scheme.
Complete the personal details declaration form
If you change jobs and continue to pay into the scheme, your employer determines if you change employment or post. If you change post, the 12-month window is not available again. Please speak to your employer.
