Your new employer should give you a Joiner Pack when you join however, you can complete it here if you have not been given one. It includes a Personal Details Declaration form to request investigation into transferring previous pension rights in to the scheme within your first 12 months of membership.
Transferring in previous pension rights
Your new employer should give you a Personal Details Declaration form to complete when you join. The form includes an option to express an interest in transferring previous pension rights into the scheme. The Personal Details Declaration form is contained in the Joiner pack (PDF, 335.7 KB).

Your cohabiting partner may be entitled to a survivors pension when you die, if they meet certain criteria. You do not need to complete a form to nominate your cohabiting partner. We will obtain the relevant information at the time of your death.
As a current member of the LGPS you can nominate any person, company, charity, club or society to receive a payment of 3 times your pay in the event of your death in service.

Join the LGPS, move to the 50/50 section , or move from the 50/50 section back to the main section. Complete a separate form for each employment which you wish to opt in or move sections. Return the form to your employer.
You cannot complete the Opt out form until you have joined the scheme. Complete a separate form for each employment which you wish to opt out of the scheme and return it to your employer. If you are opting out because you are suffering financial difficulties, you may wish to consider opting into the 50/50 section rather than opting out. View information about the 50/50 section.
To contribute to the Standard Life Additional Voluntary Contribution (AVC) scheme complete this application form detailing your investment choices and contribution amount. Return your completed form to us. We will send you an acknowledgement letter on receipt.
To contribute to the Prudential in-house Additional Voluntary Contribution (AVC) scheme complete the online application form detailing your investment choices and contribution amount.
Retirement Declaration form
You must complete this form when you retire. It is not available on this website as it is provided to you by your employer. Your employer will return it to us along with other information required to calculate your pension. If you have not been given a Retirement Declaration form to complete before your retirement date you should contact your employer directly. View information about the retirement process.
